– The FastMed Authorization for Treatment form must not be provided to an injured employee until the Employee Statement has been completed. Once the statement is submitted, the employee may proceed to a FastMed location for medical evaluation.
– A list of approved FastMed locations is included below for your reference and to assist injured employees in accessing timely care.
– Additionally, I have created an Employee Responsibilities document, which should be shared with the injured worker to help guide them through the process.
– The Student Incident Form has been updated for the 2025–2026 school year. This form is to be provided to the student’s parent or guardian, who is then responsible for submitting it directly to the insurance company.

I’m not sure what the confusion is here or what needs clarifying.
I submitted a public records request to Durham, NC Schools on 06/15/26 at 5:40pm via your public records request form. There was a very clear and specific subject and message.
Having received no response, I followed up about my request multiple times, until on July 21 @ 4:07pm, when you said: “We are in receipt of your request for information and have begun gathering the data.”
On July 29 @ 4:02pm I also received a email from Ryan stating that: “This email acknowledges receipt of your public records request received on June 15, 2026.” and giving me a reference number. “… Durham Public Schools is reviewing your request in accordance with North Carolina General Statute Chapter 132 and Board Policy 5070/7350. We will conduct a good-faith search for responsive records and provide a response as promptly as possible.”
As it’s been nearly two months since the request was submitted, can you please confirm that you actually know what my request asked for? It was submitted to you via form, and I cannot just copy and paste the message to you here. You have a record of this request being made, and apparently have been processing it since June. Although, it’s unclear why it has taken this long to complete or even begin to ask me to clarify the information I’m seeking.
Can you please explain the reason for the obvious delay in processing my request? You have been provided the exact details subject / message of the request that I submitted via your form on June 15th.

I have not received a response since submitting the request, nor to my follow up email about this on July 08.
Under NC Public Records Law, your office is required to “promptly” process all public records requests. Essentially, this means that if you intentionally delay the fulfillment of a request by allowing it to “sit” until staff has availability to address it, or feels like answering my emails, then this could be considered a denial of a request. With this in mind, I want to highlight the court case referenced in item #6 of the following article, posted on the NC Press Association website. Basic salary data is simple to provide, very commonly produced, and widely considered a matter of public record as part of the public personnel file of school employees. Many NC schools fulfill requests like this via email in 1 week, and in many cases 1 day.
Please provide the public personnel record for Stephanie Elaine Williams, Finance Department, Durham Public Schools from November 01, 2004 through March 31, 2024. Specifically – please provide the following data points:
