We received a new request through our site at www.sunshinerequest.com for information that may be available in your office.
Here are the details of the request:
Subject: Log of waiver forms for use of City of Brevard-owned exercise equipment by non-city employees
Message: “Hello Brevard City Administrators,
We request the following information that is mentioned in the City of Brevard’s contract with Trent Humphreys:
1. Exhibit A Scope of Work for contract employee/personal trainer Trent Humphreys, lists among other duties a “log of waivers for individuals using city-owned exercise equipment.” We request these logs for the years 2014, 2015, 2016, 2017, 2018 and first three months of 2019.
2. We request these logs to be released within 30 days of this request. See attachments.
3. Furthermore the above mentioned contract with Mr. Humphreys mentions a time limit of 10 hours per week for the use of city-owned facility and workout equipment for private training sessions. We’d like to request the name and position of the person that keeps track of the hours that Mr. Humphreys uses the city-owned facilities and work out equipment for non-city business.” City Manager, Jim Fatland, oversees the program and has made citywide wellness a top priority.
