(March 2024) Request for State of Florida Judiciary Courts: Current electronic copy of IRS form W-9.

Good evening Millie (or Custodian(s) of these records),

Please reply promptly to the following public records request, which I’m making in accordance with the Freedom of Information Act, the Florida Governor's Open Records Executive Order, and IRS Publication 1586.

SUBJECT:
(March 2024) Request for State of Florida Judiciary Courts: Current electronic copy of IRS form W-9.

MESSAGE:

To: JUDICIARY COURTS OF THE STATE OF FLORIDA

The disclosure of the information I’m requesting is not primarily in my commercial interest, and I believe is likely to contribute to public understanding of the operations or activities of the government, making the disclosure a matter of public interest. If your office is not the custodian of this record, please steer this request through the proper channels to reach its destination. Please reply to me within 10 days, as prescribed by IRS Publication 1589. Should your office deny this request, please state in writing the basis for the denial (including the exact statutory reference authorizing the denial).

Please provide the electronic copy of IRS form W-9 for the “Judiciary Courts of the State of Florida”. I am hoping to find out the Employer Identification Number [EIN] of this public entity.

Although I’m requesting the EIN for this particular branch of the “Judiciary Courts of the State of Florida”, I believe that there are 484 Branches and one Subsidiary operating under the same name (“Judiciary Courts of the State of Florida”) ... which may all be under one umbrella, and would mean there is only one EIN for the entire system. If this is the case, then please provide me with that EIN.

Please note that by submitting this request, I’m NOT agreeing to pay any potential fees or associated costs related to my request. If it isn’t possible to process the request without incurring fees, then please let me know the itemized potential cost and clear explanations of those charges upfront, and DO NOT proceed until I respond.

In order to move things along, I’d like to clarify that this request is being made by:

The request has been sent solely by me and my email address [email protected] . If anyone other than that email address contacts you to pursue a similar request, it did not come from me.
